Have you ever tested positive on a DOT drug or alcohol test? required
Have you ever been convicted of a felony involving a commercial motor vehicle? required
Have you ever used a commercial motor vehicle in the commission of any felony? required
Have you ever been convicted of vehicular homicide? required
Have you ever been convicted of hit‑and‑run or leaving the scene of an accident? required
Have you ever been convicted of transporting drugs or controlled substances? required
Have you ever been convicted of using a CMV in human trafficking? required
